Pino Lella a 17 ans quand la guerre rattrape cet adolescent insouciant. Il fait la connaissance d'Anna, dont il tombe aussitôt amoureux, mais elle ne le rejoint pas alors qu'il l'a invitée au cinéma et que les Alliés pilonnent la ville. Pour le protéger, ses parents l'envoient à la montagne, dans l'école du père Re, où il allait étant enfant.
Skieur émérite et bon connaisseur des chemins alpestres, Pino aide bientôt des Juifs à fuir vers la Suisse. A 18 ans, il s'engage dans l'organisation Todt, chargée de génie militaire, pour éviter l'enrôlement de force sur le front russe. Blessé lors d'un bombardement, il passe sa convalescence à Milan, où par hasard il devient le chauffeur du général Hans Leyers, bras droit d'Adolf Hitler en Italie.
Comme Pino l'accompagne dans tous ses déplacements, il est contacté par la Résistance et devient un "infiltré" au sein du haut commandement allemand. Un soir, alors qu'il conduit Leyers chez sa maîtresse, il y retrouve Anna, qui leur ouvre la porte... Pino, aux premières loges pour assister aux exactions commises par les nazis, poursuit ses missions de renseignement. Un double jeu dangereux qui le fait passer aux yeux de certains pour un collaborateur...
La fin de la guerre approche, Anna a été exécutée, et Pino doit livrer Leyers aux Américains... Sous un ciel écarlate est le récit du courage sans faille d'un jeune homme, pris dans les affres des heures les plus sombres de l'Histoire.
